Position Nurse Practitioner (NP) โ€“ Long-Term Care & Skilled Nursing

Setting Nursing home / skilled nursing facilities, with hospital coverage as needed

Location Essex County, New Jersey - travel to 3โ€“4 facilities daily

Status Full-time, five days per week

Position Overview

Nirvana Healthcare Management Services, in partnership with Salerno Medical Associates, LLP, is seeking a Nurse Practitioner to care for our patients across skilled nursing and long-term care facilities in Essex County, New Jersey. The majority of this role is based in the nursing home setting, with hospital coverage provided as needed.

This is a mobile, full-time position. The Nurse Practitioner will travel to three to four nursing homes per day, five days per week, on a flexible schedule. The role manages a broad range of clinical conditions - from long-term and post-acute care to primary care management of ventilator-dependent patients - and is ideal for an adaptable, autonomous clinician who is comfortable working across multiple sites and care teams.

Key Responsibilities

โ€ข Nursing home care: Provide direct clinical care to patients in skilled nursing and long-term care facilities, traveling to three to four nursing homes daily within Essex County.

โ€ข Complex clinical management: Manage a wide array of clinical conditions, including long-term care, post-acute and primary care needs, and the ongoing management of ventilator-dependent patients.

โ€ข Hospital coverage: Provide hospital coverage as needed to support continuity of care for our patients.

โ€ข Assessment & care planning: Perform comprehensive assessments, develop and adjust care plans, and coordinate with facility staff, attending physicians, and families.

โ€ข Documentation: Document encounters accurately and timely in the electronic health record, supporting regulatory and billing requirements.

โ€ข Collaboration: Collaborate closely with the lead Nurse Practitioner and the broader care team to ensure high-quality, coordinated patient care across all sites.

Required Qualifications

โ€ข Licensure: Active, unrestricted New Jersey Nurse Practitioner license in good standing.

โ€ข Mobility: Ability to travel reliably between three and four nursing home facilities per day within Essex County (valid driver's license and reliable transportation).

โ€ข Clinical skills: Clinical competence across a broad range of conditions in adult and geriatric populations.

โ€ข Adaptability: Comfort working autonomously across multiple sites with a flexible, full-time schedule.

โ€ข Communication: Strong communication, documentation, and care-coordination skills.

Preferred Qualifications

โ€ข LTC experience: Prior experience practicing in nursing home, skilled nursing, or long-term care settings.

โ€ข PointClickCare: Familiarity with the PointClickCare EMR system.

โ€ข Complex care: Experience managing ventilator-dependent or medically complex patients.

โ€ข Bilingual: Bilingual (English/Spanish) - a plus, but not required.
