Job description

Here's what you'll be doing:
  • Provides clinical services within the scope of a Family Practice or Pediatric Nurse Practitioner, serving the general community, but with a special focus on Pediatrics
  • Diagnoses, treats and provides high quality patient centered care for medical conditions in children and adults following accepted medical standards of practice
  • Prescribes pharmaceuticals, other medications, and treatment regimens as appropriate to assessed medical conditions
  • Refers patients to specialists and to relevant patient care components as appropriate
  • Evaluates and revises patient care/treatment plan with patient and family as necessary to achieve improved health
  • Provides education for patients and/or families about preventative care, medical issues and use of the prescribed medical treatment
  • Provides accurate documentation of patient care, document orders, directives, procedures and patient responses in the medical record and communicates them to other members of the health care team; maintain accurate, legible, timely and confidential records
  • Participates actively in the advancement of FCP's Medical Home model, including innovative ideas, function as leader of a Care Team, participation in care team clinical benchmarking, and meaningful use of the electronic health record
  • Collaborates with the Medical Director, Provider Team and Nursing Director to identify areas for system improvement, increased efficiency, communication and quality improvement activities
  • Develops relationships to provide for a broad spectrum of care with the various ancillary specialties available. (I.e. Rehab, Dental, Optical, Community Health, Behavioral Health, etc.)
  • Communicates within the department as well as across departments to ensure that information is shared for effective operations, and maintains strict confidentiality and adherence to HIPAA policies and procedures

What you'll need to be successful:
  • Master's Degree or PhD in Nursing with an emphasis in Family Practice or Pediatrics from an accredited college or training program recognized by the State of Wisconsin.
  • Possesses and maintains unrestricted licensure provided by the State of WI as an Advanced Practice Nurse, BLS certification, and unrestricted DEA licensure
  • Must possess proof of applicable health department testing and vaccinations or testing/vaccination will be provided by FCP Employee Health
  • At least 1 year of clinical practice in a Family Practice, Pediatric, Urgent Care or Emergency Department environment
  • Excellent Patient Relations and communication skills
  • Ability to participate in clinic committees as requested
  • Must successfully pass all applicable background checks and drug screens
  • FCP complies with all CDC vaccine and testing recommendations for healthcare professionals.

Preferred Qualifications:
  • At least 3 years of clinical practice in a Family Practice, Pediatric, Urgent Care or Emergency Department environment
  • Family Practice/Pediatric experience
