About the Role:
Skilled Nursing Pharmacy in Hayward, CA is looking for a dedicated and experienced PM IV Pharmacist to join our team. This is an exciting opportunity to make a meaningful impact in the lives of skilled nursing patients by ensuring safe, accurate, and timely medication management. Responsibilities:
  • Prepare, dispense, and verify IV admixtures and sterile compounding in compliance with USP 797/800 standards
  • Review and process medication orders for skilled nursing facility (SNF) patients during PM shift hours
  • Perform drug utilization review (DUR) and clinical screening for drug interactions, allergies, and contraindications
  • Collaborate with nursing facility staff to resolve medication-related issues and ensure continuity of care
  • Maintain accurate patient medication profiles and documentation in pharmacy management systems
  • Oversee and mentor pharmacy technicians during shift operations
  • Ensure regulatory compliance with state and federal pharmacy laws and long-term care guidelines
Requirements:
  • Active California Pharmacist license in good standing (RPh required)
  • Experience in IV compounding, sterile preparations, or long-term care/SNF pharmacy preferred
  • Knowledge of USP 797/800 guidelines and sterile compounding best practices
  • Familiarity with skilled nursing facility (SNF) or institutional pharmacy workflows
  • Strong clinical judgment and attention to detail in high-volume environments
