Job description

Pharmacist

The pharmacist provides advanced medication therapy management (MTM) services within referral-based Hospital Outpatient Department (HOPD) pharmacist-managed clinics. Practicing under a collaborative practice agreement (CPA) and in accordance with the California State Board of Pharmacy, the pharmacist manages complex and chronic conditions by initiating, adjusting, and discontinuing medications; conducting patient assessments; and monitoring therapeutic outcomes. The role includes collaborating with prescribers under a CPA, coordinating medication access, promoting adherence through patient education, documenting care in the electronic health record (EHR), and supporting quality improvement and professional development initiatives.

Knowledge, skills, and abilities include strong pharmacotherapy knowledge for chronic and complex conditions, understanding of state pharmacy regulations, scope of practice, and collaborative practice agreements, familiarity with evidence-based guidelines, lab monitoring, and medication therapy management, ability to assess patients, review medication regimens, and participate in development of individualized treatment plans under a CPA, supports medication initiation, modification, and discontinuation per collaborative agreement protocols, patient education and counseling, including virtual care delivery, proficiency with Epic or similar EHR systems (e.g., Epic) for documentation and tracking clinical outcomes, strong communication, problem-solving, and interdisciplinary collaboration skills, ability to adapt care delivery to diverse patient populations and telehealth environments, commitment to patient-centered care and quality improvement initiatives.

Education: Doctor of Pharmacy (PharmD) Degree from an ACPE-accredited institution required. Experience: PGY-1 Pharmacy Practice Residency or significant ambulatory clinical experience required, PGY-2 residency in ambulatory care or related specialty preferred, advanced clinical expertise in chronic disease management preferred, experience or familiarity with 340B program operations and compliance preferred, demonstrated pharmacy practice experience in ambulatory or HOPD pharmacist-managed clinics preferred. Licenses and Certifications: Current and unrestricted Registered Pharmacist (RPh) License in California required, board certification (BCPS, BCACP, or equivalent) required within 18 months of hire, current Basic Life Support (BLS) for Healthcare Providers by American Heart Association (AHA) required.
