The Clinical Pharmacist - Ambulatory Transplant is part of a specialized team of three decentralized transplant pharmacists supporting complex transplant and heart failure patients. This caregiver will primarily support the heart transplant and heart failure teams, collaborating directly with providers and participating in interdisciplinary rounds. The role also includes cross-training to provide coverage for liver and kidney transplant services, with opportunities to support inpatient clinical patients as needed.

A caregiver in this position will work Monday through Friday from 8:00am to 4:00pm, with a rotating every fourth weekend requirement. Caregivers will also work one major and one minor holiday each year. No on-call or float expectations are associated with this role.

A caregiver who excels in this role will:

  • Provide safe, efficient, patient-centeredand evidence-based pharmacy and medication services.
  • Ensure theappropriate selection, dosing,administrationand dispensing of medication orders.
  • Verify medication orders and/or dispense medications accurately according to state and federal regulations.
  • Document services in a complete,accurateandtimelymanner.
  • Actively contribute to medication safety initiatives.
  • Ensure all pharmacy operations are in compliance with national accreditation standards.
  • Educate and train students, residents, other caregivers, and/or patients.
  • Provide cost-effective care.
  • Be accountable for drug use policy adherence.

Minimum qualifications for the ideal future caregiver include:

  • PharmD or Bachelor of Science in Pharmacy from an accredited college of pharmacy.
  • Current State licensure as a Registered Pharmacist (RPh) or eligible for reciprocation in current state within90 daysof hire.
  • Individualsrequiredto administer immunizations:currentBasic Life Support (BLS)certification through the American Heart Association (AHA)or American Red Crossandcompletion of a Board of Pharmacy approved course inthe administration of immunizations or certificationobtained within90 daysof hire.
  • Individualsrequiredto respond to adult or pediatriccardiopulmonary arrest events: currentAdvancedCardiovascular Life Support (ACLS) or PediatricAdvanced Life Support (PALS)certification through the American Heart Association (AHA) or American Red Cross-orcertification obtained within one year of hire.

Preferred qualifications for the ideal future caregiver include:

  • Board certification.
  • Completion of a post-graduate year 1 residency (PGY1),fellowshipor relevant post-graduate pharmacy experience.
  • Clinical Pharmacists hired in a specialized pharmacy practice area: post-graduate year 2 residency (PGY2) in transplant specialties strongly preferred

Physical Requirements:

  • Typical physical demands include ability to perform the job while standing and walking for prolonged times, approximately 75% of shift and continuously for extended periods of time.
  • Manual dexterity sufficient to perform computer and dispensing functions of the job; ability to lift up to 30 lbs.
  • Position often includes reaching, stretching, stooping, climbing steps and sitting.
  • Must have normal or corrected vision.
  • Must have ability toclearly communicate verbally by phone or in person

Personal Protective Equipment:

  • Follows standard precautions using personal protective equipment as required.
