Building Location:32nd Avenue BuildingDepartment:3015320 CASE MGMT-SOCIAL SERVICES - 32ND HOSPJob Description:Responsible for organizing and providing care to patients through the process of assessment, planning, intervention, and evaluation. Supports the physician and interdisciplinary team to assure patient progress through the continuum of care to discharge, with the underlying objective of enhancing the quality of clinical outcomes and patient satisfaction while managing the cost of care and providing timely and accurate information to payers. The role integrates and coordinates utilization management, care facilitation and discharge planning functions. This role is accountable for planning effectively in order to meet patient needs, manage length of stay, and promote efficient utilization of resources for a designated caseload.Education Qualifications:

This position requires the ability to work the following schedule:

  • Salaried Position
  • Monday-Friday 8am-4:30pm
  • Limited weekends
  • Holiday rotation
    Licensure/CertificationQualifications:

    Certification/Licensure Required:

    • Preferred experience in Case Manager or Acute Care Nursing (2-4 years of experience is ideal).
    • BSN or ADN degree from an accredited school or college of nursing
    • Current RN licensure in appropriate state(s) of employment
    • Fargo: Basic Cardiac Life Support (BCLS) not required
