Experienced 
Unique In Home Personal Care LLC is seeking experienced, dependable, and compassionate caregivers to provide one-on-one in-home support to seniors, veterans, and individuals with disabilities throughout Dane, Jefferson, Rock, Columbia, and Janesville areas.
We're seeking caregivers with 5+ years of professional caregiving experience who are committed to delivering quality care, promoting client independence, and maintaining dignity, safety, and respect in every interaction.

Starting Pay
Starting pay is determined by verified experience, certifications, references, attendance history, and demonstrated competency.
Typical wage placement:
  • Caregiver II (5-6 years experience): $16.50-$17.50/hr
  • Senior Caregiver (7+ years experience): $17.50-$18.50/hr
Additional opportunities for advancement may be available based on performance, leadership, attendance, certifications, and agency needs.

Responsibilities
  • Assist clients with Activities of Daily Living (ADLs)
  • Provide companionship and emotional support
  • Assist with mobility, transfers, ambulation, and fall prevention
  • Prepare meals and perform light housekeeping tasks
  • Follow individualized care plans
  • Observe and report changes in client condition
  • Complete visit documentation accurately and timely using MatrixCare
  • Communicate professionally with clients, families, nurses, and office staff
  • Follow infection control and safety procedures
Physical Requirements
Applicants must be able to:
  • Stand, walk, bend, kneel, crouch, reach, and sit throughout the workday.
  • Lift, carry, push, and pull up to 50 pounds independently.
  • Assist clients with transfers, ambulation, repositioning, and fall prevention activities.
  • Safely provide physical support to clients experiencing mobility limitations or loss of balance.
  • Utilize gait belts, wheelchairs, walkers, and mobility equipment safely and appropriately.
  • Work safely in client homes with varying environmental conditions.
Required Qualifications
  • Must be 18 years of age or older
  • Eligible to work in the United States
  • Pass Wisconsin caregiver background check requirements
  • Reliable transportation required
  • Valid driver's license and current auto insurance (if driving clients or traveling between assignments)
  • Dependable attendance and punctuality
  • Ability to assist clients with activities of daily living
  • Ability to communicate professionally with clients, families, and staff
  • Ability to document electronically using MatrixCare or other approved systems
  • Reliable smartphone with stable mobile data service
  • Complete all onboarding, training, and employment documentation
  • Negative TB screening or TB test if required
