Job description

We are seeking an RN Case Manager Care Coordinator who will facilitate interdisciplinary care planning with a focus on medical necessity, patient throughput, discharge planning, and resource utilization while supporting quality patient outcomes across the continuum of care.


Responsibilities

  • Perform comprehensive psychosocial and medical assessments
  • Develop and coordinate individualized case management plans
  • Lead interdisciplinary collaboration and discharge planning
  • Conduct utilization reviews and assess medical necessity
  • Coordinate communication between physicians, patients, families, and outside agencies


Qualifications

  • Minimum 3 years of RECENT Case Manager experience in an acute care setting required (within the last year)

OR

  • 3 years of experience in Med/Surg, Tele, Neuro, ICU, PCU, or ED


Candidates with home health or insurance case management experience may also be considered if they have:

  • 3 years total acute care experience
  • At least 1 year of acute care experience within the last 5 years


Job Type: Full-Time | Permanent
Shift: Evening Shift (12:00 PM – 8:30 PM EST)
Weekend Requirement: 1 weekend per 4-week schedule
Education & Licensure

  • Associate Degree in Nursing or Nursing Diploma required
  • Bachelor’s Degree in Nursing preferred
  • Current Florida RN license or Compact RN license required
  • Case Management or Utilization Review certification preferred


Compensation

  • Salary Range: up to $43.92/hr
  • Evening Shift Differential: $2.50/hr
  • Weekend Differential: $2.00/hr
  • Weekend evening shifts receive both differentials ($4.50/hr total)


Sign-On: $10,000

Relocation Assistance: Available on a case-by-case basis
