Permanent Opportunity: PRN Family or Internal Medicine Physician Near Indianapolis, IN

We are excited to offer a PRN (Part-Time) position for a Family, Internal, or Geriatric Medicine Physician located near Indianapolis, IN. This opportunity is ideal for an experienced physician seeking to contribute to a growing outpatient PACE Program. There is potential for this part-time role to expand into a full-time position based on needs and performance.

Position Details:
  • Specialty: Family Medicine, Internal Medicine, or Geriatric Medicine
  • Position Type: PRN (Part-Time)
  • Shift: Day
  • Schedule: PRN

Responsibilities:
  • Direct Medical Care: Provide comprehensive medical care to participants in their homes, at the PACE center, and in other healthcare settings.
  • Collaboration: Work closely with the PACE Medical Director and an interdisciplinary team to ensure high-quality patient care.
  • Care Management: Develop and implement care plans with patients and their families.
  • Support Services: Utilize a network of social workers, dietitians, rehabilitation therapists, transportation services, and home health services for a holistic approach to patient care.
  • Practice Model: Operate within the PACE model, which simplifies approval processes for tests, procedures, and services as both provider and payor.
What You Can Expect:
  • Practice Hours: Monday Friday, 8:00 a.m. 5:00 p.m.
  • Schedule: Traditional PRN with flexible scheduling
  • Call Responsibilities: Based on provider staffing, with a minimum 1:3 rotation
  • Daily Duties:
    • Care planning with the PACE Program interdisciplinary team
    • Serve as the primary care physician for patients
    • Manage and round on PACE participants admitted to long-term care facilities or acute care
    • Review medical histories and assessments
    • Implement and adjust care plans in collaboration with patients and their families
    • May collaborate with advanced practice providers
  • Outreach: Potential for outreach to the Lafayette location
  • Patient Population: Skilled and long-term care patients

  • Experience: Nursing home care and Family, Internal, or Geriatric Medicine experience
