Summary/Objective

The Case Manager, as part of the Shared Services multidisciplinary team, will provide case management and advocacy services to Housing program clients.

Essential Functions

  • Manage primary case load by completing treatment plans and coordinating care through in coordination with Housing Program and Shared Services team.
  • Provide person centered life skills services to clients on an individual and group basis.
  • Assist program participants with activities of daily living.
  • Refer clients to appropriate community agencies, assisting clients when needed.
  • Act as advocate and liaison, consulting with Housing Program Managers, community agencies and program clients to maintain coordination in the care plan process.
  • Collaborate with other members of the Shared Services & Housing team during daily and weekly staff meetings.
  • Provide on-call crisis intervention in coordination with other team members, covering nighttime hours, holidays, and weekends.
  • Maintain PPCS client tracking system and client files (including treatment plans), documenting client progress to maintain a permanent record of client activity within the timeframe required by funders and partners.
  • Document all interventions within 24 hours.
  • Regular attendance and punctuality are required to perform job duties effectively.
  • Perform other duties as requested by the leadership or as dictated by circumstances of growth and/or planning.
