The position operates in a home care setting, which involves traveling to patients' residences in Fairfax and surrounding areas to deliver on-site therapy services. As a Physical Therapist, you will be responsible for:

• Directing and implementing physical therapy treatment plans aimed at preventing disability, alleviating pain, enhancing muscle function, and promoting maximum performance based on each client’s needs.
• Overseeing the tasks performed by Physical Therapy Assistants, ensuring accurate adherence to treatment plans and interventions.
• Performing comprehensive evaluations of clients to assess functional abilities using diagnostic tests focused on muscle, nerve, joint, and overall functional capabilities.
• Assessing the home environment to provide recommendations for adaptive strategies that promote client independence in daily activities.
• Administering a range of therapeutic services, including manual therapy techniques and instructional support for non-manual exercises to bolster the patient's strength and mobility.
• Monitoring client progress, recording changes in their condition, and making recommendations for adjustments in their respective care plans.
• Participating actively in interdisciplinary team meetings, contributing to a holistic treatment approach and case conference discussions.
• Educating clients and family members about the use and care of physical therapy devices as well as offering guidance on home exercise programs.

This role primarily focuses on providing rehabilitation services to a diverse clientele, including seniors, individuals recovering from surgery, and those managing chronic conditions. Emphasizing a patient-centered approach, you will assess individual needs and preferences to inform a customized treatment strategy that addresses their unique circumstances. Your work directly impacts patient mobility, independence, and overall well-being.

The ideal candidate will possess:

• A Doctorate in Physical Therapy (DPT) from an accredited institution.
• A valid license to practice as a Physical Therapist in the State of Virginia.
• Current CPR certification and a valid driver's license with automobile insurance.
• A strong foundation in clinical excellence, ethics, and a passion for delivering quality care.
• Excellent communication skills, enabling effective engagement with patients, families, and interdisciplinary team members.

While the role requires a minimum of 2 years of experience in a physical therapy setting, candidates with experience in acute care or home health will be given preference. A history of working collaboratively within healthcare teams to enhance patient outcomes will also be advantageous for applicants.

This position offers a competitive annual salary ranging from $100,000 to $120,000, commensurate with experience.
