Occupational Therapist (OT) – Skilled Nursing Facility – Pleasanton, TX

A Skilled Nursing Facility in Pleasanton, TX is seeking a dedicated Occupational Therapist to join the rehabilitation therapy team on a  day-shift schedule (36 hours per week). This role focuses on helping adult and geriatric residents regain function, maximize independence, and improve quality of life in a collaborative, patient-centered environment.​

• Complete comprehensive occupational therapy evaluations for residents in a SNF setting, including assessment of ADLs, functional mobility, and cognitive/perceptual skills.​

• Develop and implement individualized treatment plans aimed at restoring function, mobility, safety, and independence.​

• Provide evidence-based interventions such as ADL retraining, strengthening, balance and endurance training, transfer training, and cognitive/perceptual activities.​

• Collaborate closely with nursing, PT, ST, social services, and medical providers to ensure coordinated, goal-driven care.​

• Document evaluations, daily treatments, progress notes, and discharge summaries accurately and timely in the EMR or designated documentation system.​

• Educate residents, families, and caregivers on therapy goals, safety strategies, equipment needs, and home programs to support safe discharge and carryover.​

• Participate in care conferences, team meetings, and quality improvement initiatives that enhance rehab outcomes and resident experience.​

• Adhere to all facility policies, state licensure regulations, reimbursement guidelines, and professional practice standards while promoting a safe, respectful therapeutic environment.​

• Active, unrestricted Occupational Therapist license in the state of Texas.​

• Minimum of 1 year of experience as an OT; prior Skilled Nursing Facility or post-acute rehab experience strongly preferred.​

• Strong knowledge of occupational therapy treatment techniques, rehabilitation protocols, and applicable regulatory requirements in the SNF setting.​

• Excellent communication, clinical reasoning, and patient-education skills, with the ability to build rapport with residents, families, and team members.​

• Ability to work independently, prioritize a diverse caseload, and manage time effectively in a fast-paced clinical environment.​

• Commitment to professional ethics, accurate clinical documentation, and patient-centered care.​
