Job description

Description

The Chaplain - Home Health Hospice reflects the mission, vision, and values of NM, adheres to the organization's Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines, and all other regulatory and accreditation standards.

Provides or arranges for spiritual counseling and pastoral care to patients, families and staff. Assesses, treats and cares for patients' spiritual needs demonstrating an understanding of the cognitive, physical, emotional, and chronological maturation process. Responds to patients while demonstrating an understanding of the Hospice concept, policies and procedures.

A religious professional who has a combination of documented formal training in pastoral counseling and supervised counseling. Experience in a health care or clinical setting. Academic and supervised work experience must equal at least five years.  Previous experience in Hospice or Home Care preferred.

Responsibilities:

  • Evaluates spiritual needs of patients and families referred by members of the Hospice team.
  • Performs home visits to provide spiritual support and counseling services in accordance with the wishes and beliefs of the patient and family.
  • Functions as a liaison and resource to the patient's personal, spiritual, or religious advisor as requested.
  •  Works cooperatively with all disciplines involved in patient care to facilitate the interdisciplinary team approach.
  • Functions as a resource for agency staff.
  • Participates in all aspects of the Bereavement Program.
  • Participates in Hospice Interdisciplinary Group meetings, individual care conferences, staff and team meetings; also participates in QI activities as requested.
  • Assists with planning and participates in Hospice Memorial Services and annual Hospice in-service for all staff.
  • Documents care and services in accordance with agency policies and procedures.
  • Maintains and enhances professional and managerial expertise through appropriate educational/organizational activities.
  • Attends community meetings, events, and/or workshops as appropriate.
  • Demonstrates ability to care for patients with understanding of the cognitive, physical, emotional, and chronological maturation process.
  • Adheres to PCH Hospice policies regarding all aspects of HIPAA Privacy and Confidentiality Rules, dress code and attendance.
  • Performs other and related duties as directed.

Qualifications

Required:

  • Excellent verbal and written communication skills.
  • Bachelors level degree
  • One unit of ACPE Certified CPE.

Preferred:

  • Masters level degree in Theology/Ministry or related field.
