Job description

Parx Home Health Care is looking for a Registered Nurse to join our team.ย ย 

The home health nurse will be responsible for providing high quality and coordinated care to patients in their homes and communities. The ideal candidate willย  have a strong background in home health care, an understanding of state and federal regulations, and a passion for helping patients live their best lives.

What you'll be doing:

  • Conduct comprehensive assessments of patients' health status, including physical, emotional, and social factors.
  • Develop and implement individualized care plans that are tailored to each patient's needs and goals.
  • Provide patient education and support to promote self-management and prevent complications.
  • Coordinate care with other healthcare providers, including physicians, therapists, and social workers, to ensure continuity of care.
  • Administer medications, treatments, and procedures as ordered by the physician or authorized healthcare provider.
  • Document all patient care activities and communicate patient status and progress to the healthcare team.
  • Participate in quality improvement activities and initiatives to improve patient outcomes and satisfaction.
  • Maintain professional knowledge and skills by attending continuing education programs and staying up to date with the latest developments in the field.
  • Provide emotional support and compassionate care to patients and their families
  • Adhere to all applicable laws, regulations, and ethical standards of nursing practice

Requirements

What we expect from you:

  • Current and unrestricted RN license in the state of Florida.
  • Strong clinical skills and ability to perform evidenced based nursing practices.ย 
  • 1-3 years of clinical experience in a healthcare setting, preferably in home health, hospice, or community health.
  • Excellent communication and interpersonal skills with the ability to work effectively with patients, families, and healthcare providers.
  • Physical Exam and Two-Step TB Test or Chest X-Ray (no older than 6 months).
  • Yearly Mandatory in-service.
  • Proficient in using electronic health records and other healthcare technologies.
  • Valid FL driver's license and reliable transportation (Auto Insurance must also be presented)
  • CPR certification required (Original Card)
  • Level 2 Criminal Background Check
