Apollo Home Healthcare is hiring a full-time or part-time/per diem Registered Nurse (RN) to join our Home Health team, serving patients in Northern California's Bay Area. In this role, you will provide high-quality nursing services to patients in the comfort of their own homes, helping them manage their care and regain independence. You will oversee a supporting Licensed Vocational Nurse and work closely with physicians, therapists, and a supportive clinical team to coordinate care and support patients and their families.

What You'll Do
  • Provide in-home nursing visits according to the physician's plan of care
  • Assess patients' health status, including condition, history, and home environment
  • Develop and update individualized care plans based on patient needs
  • Administer medications and treatments as prescribed
  • Educate patients and caregivers on disease management and home care
  • Monitor patient progress and communicate changes to the physician and care team
  • Document visits and care in a timely manner
  • Coordinate care with therapists, aides, and other team members
  • Serve as case manager for assigned patients and support discharge planning

Requirements

  • Active California RN license
  • CPR/BLS certification
  • Strong communication and organizational skills
  • Reliable transportation and willingness to travel to patients' homes
  • Comfortable working independently in a home health setting
  • Home health experience preferred
  • Experience using Kinnser/Wellsky preferred, but not required
