Per Diem Registered Nurse (RN)

Actual Home Care LLC is currently seeking Per Diem Registered Nurse (RN) for an on-call position. Coverage is needed Monday to Friday from 9:00 AM to 5:00 PM, as well as Saturday and Sunday as required.

Position Summary

The RN will be responsible for answering calls, responding to clinical questions, and performing in-home care assessments when needed. All duties must be carried out in compliance with New Jersey Department of Health policies and guidelines, ensuring the delivery of safe, ethical, and high-quality patient care. Responsibilities include conducting patient assessments, maintaining accurate documentation per state and agency standards, and providing professional clinical guidance to patients, families, and caregivers. In the absence of the Agency Director, RN may be required to act as Director.

Qualifications
  • Current, valid Registered Nurse License for New Jersey
  • Must have a negative criminal background check
  • Must have negative TB screen
  • Must have current and valid drivers license
  • Preferable to have at least one year of hands-on assessment in a homecare setting
Duties
  • Perform in-home care assessments in accordance with New Jersey Department of Health guidelines.
  • Answer and respond to patient calls and clinical questions during assigned coverage hours.
  • Report to the Agency Director any complaints from patients, family members, or service providers, and document them accordingly.
  • Develop, implement, and evaluate individualized care plans for patients.
  • Monitor patient conditions and report changes promptly to the appropriate medical personnel.
  • Ensure compliance with all safety protocols, infection control policies, and regulatory standards.
  • Continuous availability during the hours the agency will provide services to clients.
  • Provide assistance in ensuring the agency policies are being followed by all staff.
  • Maintain confidentiality of patient information at all times in line with HIPAA regulations.
Compensation
  • $250 per weekdays (Wednesday & Friday) for on-call coverage
  • $250 per weekend for on-call coverage, Saturday and Sunday
  • $100 per initial clinical assessment and $50 for reassessment and patient visit.
