What Youโ€™ll Do

  • Conduct home visits to assess client needs, administer prescribed medications, perform wound care, and manage medical devices.
  • Follow individualized care plans and collaborate directly with the Nurse Case Manager before making necessary adjustments.
  • Measure, record, and report any deviations from a clientโ€™s baseline vital signs.
  • Identify changing health needs, advocate for increased care services or hours, and coordinate implementation with the clientโ€™s responsible party.
  • Educate clients and families on dietary recommendations, nutritional goals, and care protocols.
  • Communicate proactively with physicians and family members regarding health status, concerns, and clinical updates.
  • Provide safe client transportation and advocacy for required medical appointments as needed.
  • Complete, accurate, timely clinical documentation that reflects high professional standards.

Qualifications

  • Required License: Active, unencumbered Registered Nurse (RN) license in the State of Connecticut.
  • Required Experience: Minimum of 3 years of home health, private duty, or community-based nursing experience.
  • Vehicle & Travel: Valid driverโ€™s license, reliable personal vehicle, and active auto insurance.
