Per Diem Rn

We are currently seeking a dedicated and compassionate Per Diem RN for our Ross Adult Day Center to utilize their expertise in providing general nursing care to the participants at the Adult Day Center according to their needs.

Expected Schedule: Will range from several 8:00am- 4:30pm shifts per month.

Supervisory Responsibilities
  • In the absence of the RN Supervisor, supervises the day-to-day operations of the adult day center, the Program Aide/Van Drivers and/or the Transportation Meal Site Coordinator
  • Carry out supervisory responsibilities in accordance with the organization's policies and applicable laws
  • This position has no direct reports
Essential Responsibilities
  • Assess and monitor clients' health changes; follow-up as appropriate with family members, care givers, physicians, treatment providers and involved agencies
  • Review clients' medications and treatments as prescribed by the physician/provider, administer and document medications and treatments
  • Complete and maintain appropriate records and reporting, including resident documentation, statistical data, attendance records, and all required documentation and reporting to ensure accreditation through the Connecticut Association of Adult Day Services (CAADS); charts any concerns regarding client and keeps ADC-RN Supervisor informed
  • Provides personal care assistance to clients and assists with other program needs (i.e. meals, recreation as necessary.)
  • Screen prospective clients and collect data to determine appropriate admission to the program; coordinates admissions and discharges; assists in maintaining and updating waiting list and all other related activities
  • Treats co-workers, clients and visitors respectfully, courteously and professionally, addressing conflicts and disagreement in a positive and respectful manner.

Requirements

  • Graduate of a certified associate or bachelor's degree nursing program
  • Registered Nurse License to practice in the State of Connecticut
  • Experience with the eldercare population preferred
