All Metro Health Care, a Modivcare Personal Care Service, is looking for a Per Diem RN Supervisor Home Care. You will have the opportunity to make a meaningful impact on the lives of our clients and foster the professional growth of our home health aides.
  • Pay per Visit
    • Sup/Post Hospital: $60
    • Reassessment: $75
    • Initial Admission: $90 weekday/ $125 weekend
  • Schedule: Services will be provided two weekdays per week during daytime hours, with one of those days conducted on-site. Visits will be scheduled to coincide with the patient's aide's required presence.
  • Mileage Reimbursement: Provided at the current federal rate

You will...
  • Performs on-site supervisory and teaching visits to evaluate caregivers
  • Provides clinical orientation to caregivers on assigned home care cases.
  • Participates in the training of new nurses and in-services for field staff.
  • Performs initial nursing assessments
  • Counsels patient/caregiver in meeting nursing and related needs.
  • Assigns and coordinates services provided.
  • Assists physician in developing a plan of treatment/care and makes necessary revisions based upon patient's condition/needs.
  • Prepares clinical progress notes ensuring any changes in the patient's condition is reported
  • Monitors clinical records for complete and accurate documentation.
