Per Diem Registered Nurse

Vitality Hospice & Palliative Care is seeking an exceptional Per Diem Registered Nurse to join our team. Per Diem โ€” Every Other Saturday & Sunday

Responsibilities

As a Hospice Visit Nurse, you will:

  • Provide professional nursing care, including admissions, initial comprehensive assessments, and ongoing evaluations.
  • Develop, implement, and update individualized plans of care based on patient needs.
  • Collaborate closely with the hospice physician and full Interdisciplinary Team to ensure appropriate interventions.
  • Oversee and support the work of home health aides, social workers, and chaplains.
  • Educate patients and caregivers to promote safety, comfort, and understanding.
  • Maintain high standards of documentation, compliance, and clinical excellence.
  • Contribute to continuous improvement efforts to elevate the quality of care across the agency.
