Registered Nurse (Health Centers - Per Diem)
Job Overview
Location: This position supports outpatient behavioral health (BH) nursing team at Cascadia's 4 Health Centers across the Portland metro area (Garlington, Plaza, Talbert and Woodland Park). Cross-training to support Primary Care (PC) nursing in the same Health Centers is encouraged. This is an on-call/per diem position for all shifts. Employees must work a minimum of 4 shifts per quarter.

Position Description: The Registered Nurse is responsible for providing direct care, coordination of treatment plans, education and care of various health conditions to support the individuals we serve and their families.
The job incumbent must understand, appreciate, and respect the differences within our Cascadia community. As such, it is expected that this position promotes integrated care, our vision of trauma-informed and person-first approaches and helps create a work environment of acceptance.
Essential Responsibilities
This position description is not intended to be an all-inclusive list of responsibilities, skills, or working conditions associated with the position. Management reserves the right to modify, add or remove duties as necessary.
General
  • Formulate and implement Nursing Care Plans for medical and psychiatric conditions.
  • Follow Cascadia medication protocols while administering medications (including PRN and psychotropic) and monitor client response.
  • Collaborate with the LMP and treatment team on medical and psychiatric care plans, including timely communication related to lab results and critical updates.
  • Act as liaison with medical staff, pharmacy, labs, and external agencies for continuity of care.
  • Educate clients and families on medications, treatments, and health management.
  • Promote health education and prevention activities.
  • Educate team members on psychiatric symptoms and medication monitoring.
  • Engage families (with consent) for education on mental illness and resources.
  • Assess client behavior and collaborate on service plans addressing behavioral health.
  • Develop and implement individualized service plans using evidence-based models.
  • Coach clients in goal setting and recovery steps.
  • Coordinate with internal and external partners for optimal client care.
  • Apply Feedback Informed Treatment (FIT) practices as part of service delivery team and in alignment with Cascadia and program's current level of FIT implementation.
  • Perform other duties as assigned.

Regulatory & Compliance
  • Comply with and implement regulations, policies, and procedures under which the department and program operate.
  • Assist in crisis situations as appropriate, following emergency protocols and procedures.
  • Understand fire regulations and evacuation procedures; participate in regular safety drills in compliance with department policies and procedures.
  • Document and maintain records in a manner that ensures compliance with all agency policies and procedures and local, state, and federal regulations.
  • Maintain accurate and up-to-date documentation as required and complete electronic medical records (EMR) as required using Cascadia software.
  • Adhere to mandatory abuse reporting laws, HIPAA and 42CFR regulations requirements.
  • Complete annual employee training requirements on a timely basis.
  • Participate in all scheduled staff meetings, supervision sessions, and other departmental and agency meetings.

Qualifications
Education:
Required:
  • An active, unencumbered Oregon Registered Nurse License and CPR/First Aid Certification.
  • Must demonstrate the competencies to become credentialed as a QMHA or QMHP through MHACBO.
    • QMHA: An associate's degree in nursing from an accredited college and at least one year of mental health work experience. OR QMHP: A bachelor's degree in nursing from an accredited college or university.

Experience:
Preferred:
  • A minimum of two years of clinical experience providing inpatient or outpatient services.

Specialized Knowledge, Skills, and Abilities:
  • Effective communication and active listening skills.
  • Demonstrated effective interpersonal and customer service skills.
  • Philosophy and values consistent with a patient-centered care model.
  • Ability to work proactively and effectively in a team as well as independently.
  • Knowledge of typical medical conditions encountered by people with a severe and persistent mental illness and an understanding of the fundamental needs of those we serve.
  • Basic Knowledge of Trauma Informed Care (TIC), Motivational Interviewing, and Evidence based practices
  • Basic Understanding of recovery model, Illness management and strength-based philosophies
  • Operate computer hardware and software at a level needed to effectively perform job functions.

Other:
  • Possess a valid driver's license, one-year driving history, clean driving record, ability to pass Driver Motor Vehicle (DMV) check and complete the Cascadia online Defensive Driving course.
  • Must have the ability to perform CPR and properly use AED equipment. BLS certification training will be provided within the first 60 days of hire for client-facing staff, and CPR certification training will be provided within 120 days for all other staff. Certification must remain current.

Working Conditions
This role is centered on individuals we serve and community engagement.
Work is performed in the community, in clinic environments, and participants' home. There is regular exposure to the elements and moderate physical and environmental demands associated with this position. The work involves addressing new and unusual circumstances, requiring flexibility in time management and exercising sound judgement.
