Key Responsibilities

• Provide virtual therapy sessions both individually and in group settings that focus on mental health support and coping strategies for clients dealing with chronic illness and grief.
• Develop, implement, and adjust personalized treatment plans based on comprehensive assessments and ongoing evaluations of clients’ emotional and psychological well-being.
• Engage in regular collaboration with nurses, physicians, and other healthcare professionals within the hospice setting to ensure an integrated approach to client care.
• Maintain detailed and accurate counseling records in compliance with HIPAA regulations and company policies to support treatment continuity and legal requirements.

Patient Population / Client Focus

Your primary focus will be on supporting individuals and families who are facing end-of-life issues. This role requires sensitivity and expertise in addressing the unique emotional and psychological needs of clients, including those coping with grief and loss. Your approach will be characterized by empathy, active listening, and culturally competent interactions, fostering trust and respect with clients from diverse backgrounds.

Qualifications

• A Master’s degree in Social Work from an accredited institution, ensuring a solid foundation in clinical practices.
• An active LICSW license issued by the state of Massachusetts, demonstrating your qualification to provide clinical social work services.
• Minimum of two years of post-licensure experience in a clinical setting, preferably with exposure to hospice care or chronic illness support.
• Demonstrated proficiency in psychosocial assessments and a strong ability in developing effective, individualized treatment strategies.
• A commitment to ethical practices, with a focus on providing inclusive and culturally sensitive healthcare.

Preferred Experience

• Experience working with hospice teams or within a palliative care environment, enhancing your ability to address complex emotional and psychological needs effectively.
• Familiarity with telehealth platforms and virtual counseling techniques, which will facilitate smooth client engagement and support.
• Knowledge of community resources available to clients and their families to enhance care support and access to services.

Compensation and Benefits
