Position Overview

Care Core Psychological Center is seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join our team in a remote full-time capacity. This position will be instrumental in providing critical mental health services to individuals and families navigating end-of-life challenges within our Hospice Center in Bay Shore, New York. Key Responsibilities

• Conduct thorough psychosocial assessments, gathering essential information about clients’ mental health histories, strengths, and resources to inform treatment.
• Provide individual, family, and group therapy sessions utilizing evidence-based treatment approaches, including cognitive-behavioral therapy and trauma-focused therapy.
• Develop and implement customized treatment plans with measurable goals and interventions specific to the hospice care context.
• Collaborate closely with a multidisciplinary team, including physicians, psychologists, and nursing staff, to ensure comprehensive and coordinated care.
• Deliver crisis intervention and risk assessments as required, ensuring client safety and wellness in challenging emotional situations.
• Maintain meticulous documentation of client interactions and treatment progress in alignment with ethical standards and legal requirements.
• Facilitate support groups, psychoeducation classes, and group therapy sessions tailored to the unique challenges faced by clients and families in hospice care.

Patient Population / Client Focus

Your primary focus will be individuals and families experiencing terminal illness and the complexities associated with palliative and end-of-life care. You will play a crucial role in supporting clients' emotional and psychological needs during this sensitive time, helping them manage grief, anxiety, and depression while promoting overall well-being.

Qualifications

• A Master's degree in Social Work from an accredited institution.
• Active licensure as a Licensed Clinical Social Worker (LCSW) in the state of New York.
• At least two years of clinical experience in a mental health or outpatient setting, preferably with hospice or palliative care populations.
• Proficiency in applying evidence-based therapeutic modalities and treatment techniques specific to mental health.
• Strong interpersonal skills along with excellent verbal and written communication abilities conducive to effective client interactions.

Preferred Experience

• Experience within a hospice or palliative care team, focusing on the unique psychological needs of terminally ill patients and their families.
• Familiarity with documentation and reporting standards in healthcare settings.
• Knowledge of community resources and referral services available to support client care.

Compensation and Benefits
