• Conduct thorough clinical assessments for patients referred for hospice care, developing individualized treatment plans that reflect both patient preferences and evidence-based practices.
• Provide psychotherapy services utilizing modalities such as Cognitive Behavioral Therapy (CBT) and Dialectical Behavior Therapy (DBT), adjusting your approach based on each client's specific challenges and goals.
• Maintain accurate and timely clinical documentation in compliance with HIPAA and organizational standards, ensuring patient confidentiality and data integrity.
• Engage in multidisciplinary team meetings, collaborating with psychiatrists, psychologists, and nursing staff to create a unified care strategy that addresses both mental health needs and overall wellness.
• Participate in regular case consultations and peer review sessions to share insights, enhance diagnostic acumen, and improve treatment outcomes.
• Educate patients and families about mental health, treatment pathways, and available community resources, empowering them to make informed decisions regarding their care.
• Stay current with developments in the field through ongoing professional training, literature reviews, and engagement in relevant workshops or conferences.

Your expertise will be primarily focused on patients within the hospice care system, a population requiring sensitive, compassionate support during complex emotional and psychological transitions. • Master's degree in Social Work from an accredited institution.
