Effective communication and collaboration with colleagues, as well as with external community agencies, are fundamental to this role.

• Conduct comprehensive biopsychosocial assessments to evaluate the mental health, substance abuse, and medical needs of patients and their families in accordance with hospice protocols.
• Deliver individual and group counseling, providing support to patients and families facing emotional challenges, grief, and bereavement.
• Document clinical interactions thoroughly and timely in electronic health record systems, including mental health evaluations, progress notes, and crisis assessments.
• Identify and escalate patient needs and concerns to appropriate medical providers, ensuring that the holistic needs of patients are addressed.
• Foster strong relationships with community resources, providing patients and families with referrals to medication assistance programs and additional support services.
• Contribute to quality improvement initiatives by collaborating with interdisciplinary teams to enhance patient care delivery and outcomes.

This role primarily focuses on patients receiving hospice care, including individuals with terminal illnesses and their families. You will be responsible for addressing the complex emotional and psychological aspects associated with end-of-life care, ensuring that patients feel understood, supported, and empowered throughout their journey. Your work will also involve guiding families through the grieving process and proactively addressing their mental health needs.

• A valid and current Licensed Clinical Social Worker (LCSW) license in New York State.
• A Master's degree in Social Work from an accredited institution, accompanied by a minimum of three years of post-licensure experience in a clinical setting.
• Strong understanding of counseling techniques, crisis intervention strategies, and the ability to make informed independent judgments related to direct patient services.
• Proven experience and capability in facilitating group therapy sessions and effectively engaging with diverse populations.

• Experience managing complex clinical cases involving diverse psychosocial dynamics.
• Familiarity with electronic health record systems and data tracking methodologies for quality assurance.
• Specialized training or experience in palliative care or hospice settings.
