This role offers the flexibility of a fully remote work environment, allowing the Social Worker to deliver care efficiently through virtual platforms. The full-time schedule is structured Monday to Friday from 8:00 AM to 4:30 PM, with the potential for occasional evening and weekend hours as client needs arise. The Social Worker will maintain consistent communication with team members and clients as part of an interdisciplinary approach to care.

The Licensed Clinical Social Worker will be responsible for:

• Conducting thorough psychosocial assessments of clients, identifying emotional, mental, and social factors that may impact their overall health.
• Developing individualized care plans in collaboration with clients, their families, and interdisciplinary team members to enhance each client's well-being.
• Providing counseling and emotional support to seniors and their families, assisting them through transitions, losses, and challenges.
• Advocating for clients to ensure they access necessary community resources and maintain their rights throughout their care journey.
• Coordinating with healthcare professionals—such as nurses and caregivers—to ensure thorough and cohesive care delivery.
• Monitoring client conditions through regular virtual check-ins, staying alert for any safety or health concerns that may arise.
• Facilitating family meetings to address concerns and promote effective communication among family members regarding client care.
• Educating clients and their families about available resources, support services, and community programs that can enhance their engagement and quality of life.
• Maintaining meticulous documentation of assessments, care plans, interactions, and progress notes, ensuring compliance with ethical and regulatory standards.
• Staying updated with best practices in social work and hospice care to continuously improve service delivery and uphold organizational integrity.

The primary focus of this role centers on seniors and their families, who often face complex psychosocial challenges as they navigate aging, chronic illnesses, and end-of-life transitions. • A Bachelor's degree in Social Work from an accredited institution.
• Current and valid licensure as a Licensed Clinical Social Worker (LCSW) in Illinois.
• A minimum of two years of professional experience in a social work role, preferably within hospice, geriatrics, or community settings.
• Strong knowledge of geriatric care principles, community resources, and social work methodologies.
• Exceptional assessment, counseling, and crisis intervention skills.
• Demonstrated ability to build rapport with clients and families, exhibiting empathy and understanding.
• Proficient in electronic health record (EHR) systems and other documentation tools.

• Experience working with interdisciplinary teams in hospice or community-based settings.
• Familiarity with telehealth platforms and remote care methodologies.
• Knowledge of Medicare and Medicaid regulations related to hospice care.
