Work Environment

The full-time schedule is structured Monday to Friday from 8:00 AM to 4:30 PM, with the potential for occasional evening and weekend hours as client needs arise. The Social Worker will maintain consistent communication with team members and clients as part of an interdisciplinary approach to care.

Key Responsibilities

The Licensed Clinical Social Worker will be responsible for:

  • Conducting thorough psychosocial assessments of clients, identifying emotional, mental, and social factors that may impact their overall health.
  • Developing individualized care plans in collaboration with clients, their families, and interdisciplinary team members to enhance each client's well-being.
