This role is primarily remote, allowing you flexibility while accessing vital resources from the hospice center as needed. • Directly engage with clients in a supportive role, conducting in-depth assessments to identify mental health needs.
• Develop and implement integrated care plans that reflect the diverse needs of clients, ensuring accessibility to necessary services.
• Facilitate individual, group, and family therapy sessions focused on addressing clients’ unique challenges and promoting recovery.
• Conduct timely screenings utilizing evidence-based practices and refer clients for more extensive assessments when necessary.
• Work synergistically with colleagues to enhance team performance and patient outcomes, participating actively in team meetings and collaborative projects.
• Ensure compliance with ethical guidelines and standards of the organization, maintaining professionalism in all interactions.
• Monitor and manage a caseload effectively, tracking client progress and adjusting treatment plans as needed.
• Participate in crisis intervention and provide timely responses to emergencies, offering immediate support when required.
• Contribute to ongoing quality improvement initiatives by gathering feedback and participating in committees aimed at enhancing service delivery.
• Maintain up-to-date documentation, ensuring timely completion of reports and data submissions as per organizational standards.

Your primary clients will be individuals facing significant mental health challenges, including those residing in underserved populations. • A Bachelor’s Degree in Social Work (BSW) from an accredited institution is required; a Master’s Degree in Social Work (MSW) is preferred.
• A valid state license as a Clinical Social Worker (LCSW) in Texas.
• A minimum of two years of relevant clinical experience is essential.
• Strong interpersonal skills, enabling effective communication with diverse populations.
• Proficiency in utilizing telehealth technologies and electronic health record systems.

• Experience in hospice or palliative care settings, providing mental health support during end-of-life transitions.
• Familiarity with culturally competent practices when working with a diverse clientele.
• Demonstrated ability to work collaboratively within a multidisciplinary team.

The salary range for this full-time position is USD 70,000 to 85,000 per year, commensurate with experience.
