• Conduct thorough clinical assessments, identifying psychosocial issues impacting patients and families in hospice care.
• Create and implement individualized treatment plans that employ solution-based therapeutic interventions tailored to specific patient needs.
• Document clinical interactions and maintain accurate records using our electronic health record (EHR) systems to ensure compliance and quality care.
• Collaborate closely with a multidisciplinary care team, contributing insights to improve holistic care strategies for patients.
• Provide ongoing education and emotional support to patients and their families, guiding them through decision-making processes related to care.
• Stay informed on mental health best practices, research developments, and relevant community resources to enhance service delivery.

You will work primarily with patients receiving palliative care, their families, and caregivers who are navigating emotional challenges associated with life-limiting illnesses. Your expertise will play a vital role in improving their quality of life by helping them cope with grief, anxiety, and other emotional or psychosocial barriers.

• A Master's Degree in Social Work from an accredited institution is required.
• A valid LCSW license for the state of New York is mandatory.
• In-depth knowledge of evidence-based therapeutic interventions, particularly within the context of end-of-life care.
• Strong communication, empathy, and problem-solving abilities are essential.

• Experience in hospice care or a related field is highly desirable.
• Familiarity with electronic health records (EHR) systems and documentation methodologies relevant to social work.

• The salary range for this position is between USD 80,000 and 100,000 per year, commensurate with experience.
