As an LCSW, you will collaborate with an interdisciplinary healthcare team that includes psychologists, nurses, and medical doctors. • Engage in the development of individualized treatment plans, including discharge plans, based on comprehensive psychosocial assessments and accepted social work interventions.
• Conduct thorough assessments to identify patients' needs and perform targeted interventions, maintaining accurate, timely records as required by department policies.
• Provide counseling and crisis intervention for patients who are experiencing emotional and behavioral challenges, ensuring their immediate safety and well-being.
• Consult with families and support systems to facilitate effective treatment and discharge planning, fostering an environment of support and collaboration.
• Participate actively in team meetings to discuss treatment plans and contribute insights that enhance overall patient care.

As a Licensed Clinical Social Worker at Safe Ground, you will work closely with a diverse patient demographic, including individuals facing mental health challenges, trauma, substance use issues, and chronic stress. • Must possess a Master's degree in Social Work from an accredited program.
• Active licensure as a Licensed Clinical Social Worker (LCSW) in the state of Oregon is required.

• Strong computer skills, including proficiency in word processing, email systems, electronic medical records, and spreadsheets, are a must.
• A proven ability to work efficiently in a fast-paced environment with the capacity to manage competing priorities.

While not required, experience in a community health clinic setting or familiarity with integrated care models is preferred. Candidates who have provided services to diverse populations or have specific training in trauma-informed care will be valued highly.
