Physical Therapist
Boulder Community Health, Boulder, CO 80303
$47.28-$70.93 an hour - Per Diem - 8-hour shifts - variable days and shifts
**Range includes per diem differential. +$2.50/hr. for evening and weekend shifts.
Boulder Community Health is seeking a Physical Therapist who assumes responsibility and accountability for providing quality patient care through the provision of physical therapy services.

Qualifications:
• Current Colorado state license for Physical Therapy.
• BLS certification is required by the end of the conditional period.
• Must have experience evaluating neurologically involved patients and conditions. Developing, executing, assessing, and modifying physical therapy intervention as it applies to the neurologically involved patient population.
Responsibilities:
• Evaluative skills: Selects appropriate tests and measures to determine goals and problems, course of treatment and appropriate therapeutic procedures/modalities.
• Selects and establishes a physical therapy diagnosis.
• Planning skills: Formulates and maintains an individualized patient plan of care based on long and short-term goals.
• Implementation skills: Provides treatment using appropriate tools and demonstrates a complete knowledge of patient management.
• Assessment skills: Evaluates patient response to treatment and revises plan of care as indicated.
• Serves the patient population from adolescent to older adult/geriatric. Is able to modify assessment, treatment and interaction skills based on the age and developmental level of the patient.
• Promotes effective communication and teamwork with team members, the hospital and other contacts.
• Reports and documents evaluations, plans, treatment techniques, assessment, and patient progress.
• Initiates and is responsible for problem-solving and decision-making with relation to patient care.
• Functions as an advocate for the patient
