Job description

SUMMARY:
As a Speech Pathologist, your empathy, compassion, and exceptional evaluation abilities lead to rehabilitative treatment programs that restore or improve patients' optimal level of function, independence, and quality of life.
STATUS: Per Diem
LOCATION: Golisano Rehabilitation - 1555 Long Pond Rd. Rochester, NY
DEPARTMENT: Rehabilitation/Speech Pathology
SCHEDULE: Day shift, 8-hr. shifts, 8:30 am - 4:30 pm (per diem as assigned)
occasional weekends & holidays
RESPONSIBILITIES:
  • Evaluation & Planning. Perform a complete initial evaluation of the referred patient; develop and implement treatment plans, including appropriate short- and long-term goals; involve the patient, family, and staff in the implementation of the treatment program; re-evaluate patient functional status progressively; advance patient toward maximal performance and functional independence
  • Documentation. Maintain a complete documentation of patient evaluations, treatment, progress, and discharge status; send required information to appropriate professionals
  • Collaboration. Actively participate in rehabilitation team efforts to meet patient needs, including counseling and teaching patient and family, discussing
  • changes in treatment programs with MDs, coordinating healthcare plans with other involved professionals, and assisting in discharge planning as appropriate

REQUIRED QUALIFICATIONS:
  • Master's degree in Speech-Language Pathology or Communication Disorders (may be MA, MS, Med)
  • Required Licensure/Certification Skills: New York State Licensure
  • Minimum of nine (9) months of supervised work experience (completion of CFY), or participation in supervised clinical fellowship, for a minimum of 36 weeks under a NYS licensed Speech Pathologist, followed by the NYS licensure examination and achievement of NYS license
  • Compassionate, warm, and patient-focused
  • Excellent organization and planning skills

PREFERRED QUALIFICATIONS:
  • One (1) year of experience with the frail elderly population is preferred

PHYSICAL REQUIREMENTS:
M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.
