Pharmacist Per Diem - Job Overview

Schedule: Monday-Friday (7:30AM-8PM) | Saturday (7:30AM-5:30PM) | As Needed

Key Responsibilities
  • Interpret, clarify, and transcribe medication orders accurately and timely.
  • Ensure the appropriateness of all medication orders with regards to client diagnosis and interactions.
  • Review patient profiles for appropriate drug selection and dosage.
  • Conduct Drug Utilization Reviews to check for drug duplication and interactions.
  • Document adverse drug reactions and interactions with patients and healthcare providers.
  • Dispense medications and maintain accurate inventory records as required.
  • Promote evidence-based medicine by staying updated on new drugs and medical literature.
  • Advise patients on health, illness prevention, and medication adherence.
  • Provide drug information and education to patients and healthcare providers.
  • Demonstrate the ability to set priorities and work cooperatively within and outside the department.
  • Function as a primary resource for patient inquiries, ensuring high customer service standards.
  • Understand financial reimbursement and special billing conditions.
  • Display compassion and sensitivity in interactions with patients and healthcare staff.
Minimum Qualifications
  • Graduate of an accredited college/university of pharmacy (Pharm. D or B.S. with equivalent experience).
  • Certification in immunization delivery, smoking cessation, and naloxone (or eligible).
  • Valid license from the Board of Pharmacy of the State of Texas.
  • Valid Texas Pharmacist Preceptor license.
  • Must maintain First Aid /CPR.
  • Provider shall obtain, maintain and participate in the Medicare and Medicaid Programs, workers compensation, other federal and state reimbursement programs, and the payment plan of any commercial insurer, health maintenance organization, preferred provider organization, accountable health plan, and other health benefit program requested by Legacy.
  • 2+ years of healthcare experience.
  • 1+ years of community pharmacy experience, preferred
  • 1+ years of medication adherence counseling experience, preferred
  • 1+ years of Medicare market experience, preferred.
  • Experience in HIV/AIDS, Geriatrics, and/or Pediatrics, preferred.
  • Computer experience Windows, Excel, and Accounting software.
  • Must be able to manage multiple deadlines and prioritize.
