LPN Licensed Practical Nurse Per Diem

Job Summary:

We are seeking a compassionate and experienced LPN to join our team at Rutland Center for Living and Rehabilitation, a premier residential, long-term, rehabilitation, and subacute care provider. As an LPN Per Diem, you will play a vital role in delivering exceptional patient care and supporting our mission to "Revolutionizing Care Delivery." As part of our per diem team, you will work on an as-needed basis, providing flexible and responsive care to our residents and patients.

Responsibilities:

  • Provide high-quality, patient-centered care to residents and patients, including administering medications, wound care, and other treatments as needed

  • Collaborate with interdisciplinary teams to develop and implement personalized care plans, ensuring coordinated and effective care delivery

  • Conduct routine vital sign checks, monitor patient conditions, and report concerns to RNs or other healthcare professionals
