What You Will Do As a vital member of our home health team, you will:

  • Provide skilled nursing care tailored to each client's unique needs

  • Deliver G-tube, trach, or vent support when required
  • Educate and guide clients and families to support ongoing health at home
  • Communicate effectively with the care team to ensure continuity of care

Who We Are Looking For • Licensed as an LPN and in good standing with the Georgia Board of Nursing • Experienced in a clinical setting, with at least one year preferred • Certified in CPR • Skilled in EMR documentation, with strong organizational ability • Confident working independently in a home environment • Compassionate, client-focused, and a strong communicator • Experienced with G-tube, trach, or vent care • Licensed to drive with reliable transportation
