Job Summary

As a Hospice RN, you'll manage a caseload of patients working as part of a dedicated interdisciplinary team. You'll assess, plan, and coordinate care while monitoring and adjusting treatment as needed to promote the best possible outcomes.

Duties & Responsibilities

As a Hospice RN, you'll manage a caseload of patients working as part of a dedicated interdisciplinary team. You'll assess, plan, and coordinate care while monitoring and adjusting treatment as needed to promote the best possible outcomes.

Your Day-to-Day Might Include:

Performing comprehensive assessments and developing personalized care plans Coordinating care and referrals across disciplines and specialties Monitoring patient progress and adjusting care to achieve optimal outcomes Communicating with patients, families, and providers to ensure continuity of care Educating patients and caregivers on managing conditions at home Documenting clinical notes and updates with accuracy and clarity Identifying and addressing opportunities for care improvement

Requirements
