Registered Nurse (RN) – Per Diem Medicare Home Health | OASIS Experience

Dedicated Home Health is growing in Palm Beach County and is seeking experienced Registered Nurses (RNs) to join our Medicare Home Health team on a per-diem basis.

What We're Looking For

Medicare Home Health and OASIS experience are strongly preferred.

Qualified candidates should have:
  • Current, unrestricted Florida Registered Nurse (RN) license
  • Medicare-certified Home Health experience
  • Hands-on OASIS assessment experience
  • Experience completing Start of Care (SOC), Resumption of Care (ROC), Recertification and Discharge visits
  • Strong head-to-toe assessment and clinical documentation skills
  • Understanding of Medicare home health requirements and plan-of-care coordination
  • Ability to communicate effectively with physicians and the interdisciplinary care team
  • Reliable transportation and ability to travel within the assigned territory
  • Commitment to timely documentation and high-quality patient care
The Role

As a Per Diem Home Health Registered Nurse, you will provide skilled nursing care to patients in their homes while coordinating care with physicians, therapists and other members of the interdisciplinary team.

Responsibilities may include:
  • Completing comprehensive OASIS assessments
  • Start of Care and other skilled nursing visits
  • Medication reconciliation and patient/caregiver education
  • Disease-process and chronic-condition management
  • Wound assessment and treatment, when ordered
  • Monitoring changes in patient condition
  • Communicating significant findings to physicians and the clinical team
  • Updating and coordinating the patient’s plan of care
  • Completing accurate and timely clinical documentation
