Hospice Homecare LPN - Per Diem Saratoga&Washington Counties

WEEKDAYS 8am to 4pm

The Community Hospice has a wonderful opportunity for a Per Diem LPN to workas a member of our Hospice Home Care Team caring for patients and families in both the homecare and nursing home settings. Primary region to cover is Saratoga and Washington Counties.

Needs would be on the WEEKDAYS, no weekends

Position Highlights:

  • NO WEEKENDS, NO ONCALL
  • National pre-taxed mileage reimbursement
  • Meetings and charting conducted via supplied lap top
  • Comprehensive orientation provided
  • Prioritizing Your Safety:Ensuring your safety is our top priority. From comprehensive training to providing necessary protective equipment, we're committed to creating a secure environment for you as you deliver exceptional care in our clients' homes.

What you will do:

  • Coordinate closely with ourRN Supervisor and RNs to provide nursing and personal care services as a part of a comprehensive plan of care to provide added support to patients and families
  • This is an ideal opportunity for someone who enjoys working closely with a team (our teams include RNs, Aides, medical directors with specialized expertise in end of life care, social workers, chaplains, complementary therapists and volunteers).
  • Training provided in end of life care.

What you will need:

  • Valid NYS LPN License
  • Minimum 1 year LPN experience required
  • Prior hospice/ home care experience preferred, but not required
  • Strong computer skills
  • Organization and communication skills and ability to work as part of a team

ENHANCED RATES FOR PER DIEM STATUS

Pay Range for non-per diem status: $25.00-$31.10 Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location."
