Per-Diem Nurse Practitioner

La Clínica del Pueblo (LCDP) is a community-based Federally Qualified Health Center rooted in health equity and culturally responsive care. We provide comprehensive, integrated primary care, behavioral health, and a range of services to communities that have historically faced barriers to care. Our model of care is team-based and relationship-centered. Primary care clinicians work alongside nursing, behavioral health, case management, health education, and community services to deliver whole-person care that addresses both medical needs and social drivers of health.

The Per Diem Nurse Practitioner provides comprehensive primary care services to patients across the lifespan within a mission-driven, community health setting. Working in close collaboration with interdisciplinary clinical teams, the Per Diem NP delivers high-quality, evidence-based care while contributing to a culture of continuous improvement, collaboration, and equity.

Required
  • Master's degree in Nursing.
  • Board Certification as a Nurse Practitioner.
  • Active or eligible licensure in the District of Columbia and Maryland.
  • 2+ years experience providing primary care as a Nurse Practitioner.
  • Experience using electronic health records in ambulatory settings.
Preferred
  • Experience working in FQHCs or community health settings.
  • Bilingual in Spanish and English.
Clinical Care
  • Provide comprehensive primary care services, including preventive care, acute visits, and chronic condition management on a per diem basis, covering existing primary care clinicians' panels.
  • Conduct clinical histories, physical examinations, assessments, and diagnostic evaluations; develop and implement appropriate treatment plans.
  • Deliver preventive health screenings, health maintenance, and patient education tailored to individual and community needs.
  • Provide culturally responsive, trauma-informed, and linguistically appropriate care.
  • Document clinical encounters accurately and efficiently in the electronic health record.
  • Review and incorporate outside records, consultant reports, and diagnostic results into clinical decision-making.
  • Coordinate referrals and additional services including behavioral health, nursing, case management, and specialty care.
Team-Based & Integrated Care
  • Collaborate closely with interdisciplinary care teams to support whole-person care.
  • Participate in regular multidisciplinary team meetings and shared care planning.
  • Provide clinical guidance and informal mentorship to staff as appropriate.
Teaching & Training
  • Participate in continuing education and in-service programs.
  • Participate in training to enhance ability to care for specific populations and conditions.
Administrative
  • Participate in clinical quality improvement and risk management activities in collaboration with the clinical team.
  • Document appropriate clinical information in the electronic health record in a timely manner, completing documentation of clinical encounters and locking progress notes within 48 hours.
  • Meet regularly with supervisor to report and review clinical performance, documentation, quality of care measures, patient feedback and other topics as determined by supervisor.
  • Abide by La Clínica del Pueblo's policies, procedures, and requirements for clinical staff, including but not limited to those pertaining to clinical documentation, billing and coding, protected health information, and communications.
  • Perform other relevant activities as assigned.
