Job description

Per Diem Opportunities Across the Entire Bay Area

(San Francisco, Peninsula, Santa Clara, San Jose, East Bay)

About the Company

At AlphaCare Home Health, we believe patients deserve more — more time, more communication, and more care that actually changes outcomes.

While many agencies use technology to increase visit volume, we use it to give clinicians more time with each patient. We are building a home health organization centered on clinical quality, meaningful patient relationships, multilingual community care, and strong operational support for the clinicians delivering care every day.

About the Role

We are seeking skilled and compassionate Speech-Language Pathologists (SLPs) to join our team on a per diem basis across the Peninsula and South Bay. The ideal clinician is patient-centered, clinically strong, and focused on improving communication, cognitive function, and swallowing safety in the home environment.

As an AlphaCare SLP, you will evaluate and treat speech, language, cognitive-communication, and swallowing disorders for patients recovering from illness, injury, or neurological conditions. You will develop individualized treatment plans, provide targeted therapy, and ensure coordinated care alongside physicians, nurses, and interdisciplinary team members.

At AlphaCare, success is measured not only by compliance — but by functional communication, swallowing safety, independence, and quality of life.

Qualifications

  • Master’s degree in Speech-Language Pathology from an accredited program required
  • Current California SLP license required
  • Certificate of Clinical Competence (CCC-SLP) preferred/required per assignment standards
  • New graduates are welcome to apply
  • Strong clinical assessment and treatment planning skills
  • Excellent communication, documentation, and interpersonal skills
  • Ability to work collaboratively with patients, families, and interdisciplinary teams
  • Multilingual abilities are strongly valued

Responsibilities

  • Evaluate patients in the home setting for speech, language, cognitive, and swallowing disorders
  • Develop and implement individualized treatment plans based on physician orders
  • Provide therapy for communication, cognition, voice, and dysphagia management
  • Assess swallowing safety and recommend strategies to reduce aspiration risk
  • Train patients and caregivers on communication strategies and swallowing precautions
  • Monitor, document, and communicate patient progress and clinical changes
  • Collaborate with physicians, nurses, and care team members to coordinate treatment
  • Participate in case conferences and care planning discussions
  • Reassess patient needs and adjust treatment plans as appropriate
  • Maintain accurate, timely, and compliant clinical documentation
  • Support safe discharge planning and functional independence goals

Why SLPs Choose AlphaCare

  • More time with patients, not pressure for maximum visit volume
  • AI-supported documentation designed to reduce administrative burden
  • Strong clinical and operational support so you can focus on therapy, not logistics
  • Flexible per diem scheduling across Peninsula and South Bay territories
  • A multilingual care environment that reflects the communities we serve
  • Leadership focused on clinical quality, responsiveness, and patient outcomes
  • A team culture built around respect, accountability, and meaningful patient progress

Benefits

  • Mileage, parking, and bridge toll reimbursement
  • Flexible per diem scheduling
  • Clinical team support structure

Work Location

In-person home visits across assigned territories

Pay Range

$70.00 – $80.00 per hour
