As a Primary Care Physician (PCP), you will lead a team that includes Nurses, Medical Assistants, Patient Care Coordinators, Case Managers, Social Workers, Community Outreach Specialists, and Consulting specialists, all working together to provide excellent care for our patients.
Our Centers also feature convenient services for our patients at no cost to them, including transportation through Uber and Lyft for appointments. Additionally, our Community Outreach Specialists organize engaging activities such as Cooking Classes, Chair Yoga, Silver Sneakers, Senior Prom's, Birthday parties, Wellness Classes, and more to help keep our Senior Patients active and connected.
The Centers are welcoming and inviting, complete with a coffee bar, event spaces, and lots of room for patient care.
