Patient Safety Manager – Falls Church, VA

Position Type: Full-Time | Exempt

Salary Range: $96,000-$120,000

Dominion Hospital, the only freestanding mental health facility in Northern VA, is seeking an experienced Patient Safety Manager to support the implementation and daily operations of the Patient Safety Program under guidance of the Director of Quality and Risk Management. This role assists with coordinating safety initiatives, analyzing safety events, and supporting compliance efforts while ensuring alignment with facility standards, CMS, Joint Commission, and state regulations.

What You'll Do
  • Support the Director of Quality and Risk Management in maintaining the Patient Safety Program and Plan
  • Manage event reporting systems and ensure timely follow-up
  • Ensure serious events are reported to the respective agencies in a timely manner
  • Conduct and coordinate RCAs, ACAs, and FMEAs
  • Analyze trends in safety events and prepare reports for review by the Director
  • Assist with regulatory readiness activities and survey preparation under Director leadership
  • Compile patient safety data and dashboards for Director presentation to leadership committees
  • Promote a Just Culture environment
  • Collaborate with interdisciplinary teams while maintaining clear escalation to the Director
  • Investigate and follow-up on patient complaints and grievances
  • Act as patient advocate
  • Support initiatives related to suicide and self-harm risk reduction
  • Assist with ligature risk assessments and mitigation tracking
  • Participate in restraint and seclusion reduction efforts
  • Contribute to patient aggression and staff safety initiatives
  • Reinforce trauma-informed care practices
Qualifications
  • Education: Bachelor’s degree required; Master’s preferred
  • Licensure/Certifications: RN or MSW preferred; CPPS or CPHQ preferred
  • Experience: 3–5+ years in patient safety, quality, or risk management, preferably in behavioral health or acute care
  • Experience with performance improvement methodologies (Lean/Six Sigma preferred). Knowledge of HRO principles.
