PCC Essential Job Duties:

1.   Development, implementation and facilitation of patient outreach for quality improvement programs.
2.   Review, auditing and reconciliation of quality data reports and related clinical documentation.
3.   Contacting patients to schedule appointments, testing.
4.   Contacting patients to follow-up on the status and assisting in re-scheduling of missed appointments.
5.   Complete responsibilities based upon established protocols and work with multiple providers.
6.   Performs other job related duties as assigned.
7.   Be a positive representative in the promotion of our Quality Program for the Clinic.
8.   Performing Medicare annual wellness visits on patients.
9.   Complying with CMS guidlines for AWV's.
10. Helping patients stay up to date on preventative services.
11. Data migration for preventative services.
12. Must be able to multi-task
13. Must stay abreast of organization/billing protocols in relation to annual wellness visits.

Education, Training & Experience

1. High school graduate/GED.
2. Graduate from an accredited Medical Assistant or Licensed Practical Nursing program.
3. Currently registered with the Indiana State Board of Nurses Registration and Nursing Education as a Licensed Practical Nurse. Or active certification for Medical Assistant (AAMA or NHA).
4. Basic knowledge of medications recommended by nationally recognized quality measures is needed.
