Job description

Lake Regional Health System is looking for a dedicated, professional Patient Portal Coordinator to join our Health Information Management (HIM) team. With our state-of-the-art hospital and network of clinics, therapy sites, and pharmacies, Lake Regional offers a wide range of employment opportunities in our community.
Principal Accountabilities and Essential Duties of the Job
  • Interacts with requestors to identify medical records needed for patient care, review, billing, or release of information.
  • Prioritizes release of information requests, retrieves medical records from active, inactive, and interdepartmental locations, electronically tracks and delivers records in accordance with established procedures.
  • Performs self-quality checks on all work to assure accuracy of the release and confidentiality of proper invoicing.
  • Handles all requests and inquiries for patient health information, whether received via mail, fax, phone, or in person.
    • Verifies the patient's identity and confirms that the authorization is valid.
    • Ensures the requesting party has a legal right to request a patient's medical information.
  • Access computerized patient records and/or other computer systems for patient information needed for faxing and for other requestors of the medical record.
  • Works with Release of Information Vendor, Ciox. Communicates with Ciox on releases, updates and logging. Will work in Health Source for logging, monitoring releases, and communication with Ciox.
  • Is a subject-matter expert on Portal functionality and registers new patients daily to the LRHS Patient Portal and the LRMG Patient Portal.
  • Interacts with patients directly to provide education on the benefits of Portal access and usage, as well as staff.
  • Collects and/or follows up with patients to capture consent for enrollment
  • Ensures appropriate LRHS/LRMG Portal patient/users are linked to the correct medical record(s)
  • Assists patients in the enrollment and initial log-in process for the LRHS Portal. This includes working with patients in the hospital prior to discharge and calling patients to assist them after discharge.
  • Serves as a primary resource/liaison to assist hospital personnel and patients through the patient establishment process.
  • Directs all secure messages to the appropriate provider and/or staff for the LRHS Patient Portal
  • Serves as the primary technical contact for patients using the LRHS Patient Portal
  • Assists in collecting and reporting data regarding Portal usage
  • Monitors daily activity on the Portal
  • Participates in LRHS/LRMG activities promoting the Patient Portal and Health Information exchange
  • Assists in developing recommendations to maintain an efficient and effective Portal administration process.
  • Performs at established quantitative and qualitative work standards to meet departmental goals and objectives.
  • Promotes quality improvement, staff and patient safety, and cultural diversity through departmental operations and through personal performance.
  • Provides service in a manner that is appropriate for the patient's age; demonstrates knowledge and skills necessary to meet the patient's physical, psychosocial, educational, and safety needs.
  • Presents a courteous and helpful demeanor, appropriate for age, to all patients, visitors, other LRHS employees/medical staff, or any other person an employee encounters while representing the health system.
  • Maintains current knowledge related to applicable statutes, regulations, guidelines and standards necessary to perform job duties in accordance with the requirements of the Corporate Compliance Plan.
  • Understands and follows all hospital and departmental policies and procedures
  • Attends departmental meetings, TJC required workshops, and other in-service seminars as appropriate for job functions. Follows hospital safety rules.
  • Contributes to the identification and solution of problems. Maintains strict confidentiality of patient and hospital information. Qualifications:

High School Diploma or equivalent required
Preferred RHIT Certification
Additional certifications may be required for this position and will be coordinated by Lake Regional as a part of your ongoing training and development.
An annual influenza vaccination is required.
