We currently support over 9,000 patients across 25+ practice locations, including partnerships with Corporate Medical Providers across the Carolinas and Virginia. What you'll do day-to-day:

  • Reach out to your assigned patients on a structured schedule — some weekly, some monthly — based on their condition and risk level
  • Review remote monitoring dashboards and proactively contact patients when data is trending in the wrong direction
  • Help patients navigate real-world barriers: late refills, insurance questions, transportation, specialist coordination, copay assistance
  • Document patient interactions using AI-assisted tools (less typing, more connecting)
  • Escalate clinical concerns to an on-team pharmacist using clear protocols
  • Assist to enroll newly eligible patients into the program

Requirements:

Qualifications Needed:

  • Certified Medical Assistants (CMA, CCMA, RMA, NCMA, or equivalent national certification)
  • Licensed Practical Nurses (LPN) with an active NC license
  • Registered Nurses (RN) looking to step away from bedside and into a structured, relationship-based model
  • Pharmacy technician (Certified)
