We are seeking a compassionate and skilled Registered Nurse (RN) who is passionate about delivering high-quality patient care in the home setting. Employment Type: Part-Time/ Per-Diem (Flexible Scheduling Available)
Compensation: Competitive Pay (starting at 50.00 per hour)
What You Will Do:
  • Conduct Start of Care (SOC) visits to complete comprehensive clinical assessments and develop individualized, physician-directed care plans
  • Perform skilled nursing visits and ongoing reassessments to monitor patient condition and adjust care plans as needed
  • Provide hands-on skilled nursing care, including wound care, medication management, injections, and chronic disease management (for skilled nursing only)
  • Deliver patient and caregiver education on diagnoses, medications, and preventive care to reduce hospitalizations
  • Collaborate with physicians and interdisciplinary team members to ensure coordinated, high-quality care
  • Maintain accurate and timely clinical documentation in compliance with state regulations and agency standards
  • Supervise and delegate care to LPNs and Home Health Aides to ensure quality and adherence to the plan of care
What You Will Have:
  • Active Registered Nurse (RN) license in the state of practice
  • Experience in home health, skilled nursing, or a related healthcare setting preferred
