Job description

Bright Paths Home Health Agency is searching for a Registered Nurse (RN), the "Heartbeat" of any healthcare organization. Their skills and compassion will contribute to the health and well-being of countless patients. A qualified admissions Nurse that will collaborate with state and local physicians to adhere to regulatory compliance in the state of Maryland. This nurse will evaluate incoming patients to determine the severity of illnesses if applicable, and the intensity of services they will require while in our care. This Nurse will be the first caregiver that spends time with our clients collecting medical data including history with the intent to create an individualized plan of service. 

Initial Assessment: Gather information about patients, medical history, current condition, medications, lab results and other relevant data.

Nursing Diagnosis: Identify the patients specific need and potential risk based on the assessment findings.

Goals: Set measurable and achievable goals for the patients care, focusing on desired outcomes

Interventions: Detailing the specific actions nurses and caregivers will take to address the patient's needs and steps to achieve the established goals.

Evaluation: Regularly assessing the effectiveness of the interventions and adjusting the plan of service as needed based on the patient's progress.
