Job description

RN Case Managers are a critical part of the agency and must have experience working in the home health setting. We are looking for individuals that are versatile and dependable. Individuals must have the following:
  • Must hold a current license as a Registered Nurse from the Virginia State Board of Nursing.
  • Must have experience working in a home care agency.

Job Duties:
  • Complete initial and ongoing comprehensive assessments for client admissions.
  • Create patient plan of care and reevaluate as necessary.
  • Identify any additional patient needs and facilitate appropriate referrals.
  • Complete clinical nurse notes in their entirety.
  • Facilitate monthly home visits for skilled care.
  • Additional duties as assigned.
