Job description

RVNA Health is seeking an Extended Hours Coordinator to join our Intake Coordination team on a part-time basis. This role is vital in supporting our Home Health and Hospice services during weekend and holiday hours.Schedule needed: Saturday & Sunday, 8:30 AM - 4:30 PM. Also required are 4 holidays per year.Work Location: This position is remote from home during scheduled weekend and holiday hours, however, the incumbent must reside within a reasonable commuting distance of RVNAhealth's Ridgefield, CT 06877 office and be able to report onsite for required training, meetings, or other business needs as requested.Key Responsibilities:Coordinate all after-hour and weekend home health and hospice referrals.Schedule patient visits based on physician orders and update schedules accordingly.Answer incoming calls from patients, families, and referral sources, providing timely support and information.Communicate and collaborate with the Extended Hours Triage RN, field clinicians, and referral sources.Enter and manage patient intake information, insurance verification, and initial authorizations.Ensure compliance with all regulatory guidelines, including visit reconciliations and proper documentation.Support weekend office functions including intake processing and communication with clinical teams.Escalate potential issues to appropriate clinical and administrative leaders as needed.Maintain a strong understanding of payer requirements (Medicare, Insurance, Title 19).Uphold agency policies and actively contribute to ongoing quality improvement initiatives.Qualifications:Required: Minimum of 2 years' experience in healthcare (familiarity with medical terminology).Preferred: Background in community health, home health, or hospice with regulatory knowledge.Exceptional organizational skills and the ability to handle multiple priorities with calm and efficiency.Excellent customer service skills with strong phone communication and interpersonal abilities.Computer literate with the ability to navigate electronic systems and enter data accurately and timely.Must be available to work all weekends and four holidays per year.Must reside within a reasonable commuting distance of RVNAhealth's Ridgefield, CT 06877 office and be able to report onsite for required training, meetings, or other business needs as requested.Why RVNAhealth?RVNAhealth (formerly known as RVNA, the Ridgefield Visiting Nurse Association) is a twelve-time winner of the Hearst Media Top Workplaces Award.
