Certified Nurse Aide (CNA) – Home Health & Hospice


Full-Time / Part-Time Positions Available


Job Summary:

We are seeking compassionate and reliable Certified Nurse Aides (CNA) to join our Home Health and Hospice team. The CNA will provide high-quality, patient-centered care to clients in their homes or hospice setting under the supervision of a Registered Nurse (RN) or Licensed Vocational Nurse (LVN).
The ideal candidate is dependable, caring, and committed to supporting patients with dignity and respect during their care journey.

Key Responsibilities:


  • Provide personal care services including bathing, grooming, dressing, and toileting
  • Assist patients with mobility, transfers, and ambulation
  • Monitor and report changes in patient condition to supervising nurse
  • Assist with vital signs (temperature, pulse, respiration, blood pressure)
  • Support patients with activities of daily living (ADLs)
  • Maintain a clean, safe, and comfortable environment for patients
  • Follow care plans as directed by the RN/LVN
  • Document care provided accurately and timely
  • Comply with all agency policies, HIPAA, and safety standards

Education & Certification Requirements:


  • Must be a Certified Nurse Aide (CNA) with a valid CNA license in the state of Texas (or applicable state)
  • High School Diploma or GED preferred
  • Current CPR certification (preferred or required depending on agency policy)

Experience Requirements:


  • Prior experience in Home Health, Hospice, Nursing Home, or Hospital setting preferred
  • Experience working with elderly, disabled, or terminally ill patients strongly preferred
  • Familiarity with ADLs and patient care documentation

  • Strong communication and interpersonal skills

  • Ability to work independently in patient homes
  • Reliable transportation and valid driver’s license (if required for field visits)
