Job description

TITLE: Transportation Driver

REPORTS TO: Administrator/ Transportation Supervisor

METHOD OF PAY: Hourly

PURPOSE OF POSITION:

The primary purpose of your job position is to provide transportation in a safe and timely manner, in accordance with established policies and procedures, and as directed by your supervisor.

ROLE and RESPONSIBILITIES:

  1. Communicate with dispatcher on Two Way Radio
  2. Be able to operate a Hydraulic Lift.
  3. Report to Dispatcher and pick up patient schedule daily.
  4. Transport 5 - 6 patients per day to and from doctors' appointments.
  5. Inspect van daily. (ex: check oil, tires, first aid kit & fire extinguisher)
  6. Ensure that all passengers are secured by seat belts.
  7. Complete all necessary forms, (ex: DMA 408, van log, etc.)
  8. Collect co-payments and private payments from patients
  9. Other related duties and responsibilities that may become necessary to assure that the needs of the facility are met.

KEY CHARACTERISTICS:

  • Highly focused and strong attention to detail
  • Ability to follow written and oral instructions
  • Ability to deal tactfully with personnel, residents, visitors and the general public.

EDUCATION/EXPERIENCE/QUALIFICATIONS:

  1. Be a minimum of 25 years of age and of good moral character.
  2. Must have CPR and First Aid License.
  3. Must have completed the Police Traffic Course.
